Benefits are dependent upon whether an individual is working full or part-time and are subject to change.  The benefits listed may not be all inclusive depending on the position.

Medical/Dental
Insurance
Cap on single plan, employee pays portion.  Cap on family plan, employee pays portion.  Eligible first of the month following 90 days.  There is no office co-pay if an employee sees an MMH&C Physician/Nurse Practitioner. 
Vision Insurance Benefit illustration sheet available.  Eligible first of the month following 90 days.
Life Insurance 

Life Insurance (Term): Group term life and accidental death and dismemberment insurance.  Eligible first of the month following 90 days.

Dependent Life Insurance Under Group Term: Dependent life insurance coverage.  Eligible first of the month following 90 days.

Supplemental: Supplemental life insurance.  Eligible first of the month following 90 days.

Whole Life Insurance (Voluntary): Whole Life - Employee owned policy.  Eligible first of the month following 90 days

Flexible Benefit Plan Ability to pay certain expenses with before tax dollars.  Eligible first of the month following 90 days.
Retirement Plan

TSA (403b Plan): Shelter a percentage of your income.  Eligible immediately.

Pension Plan: Hospital will match 2% of your voluntary contribution providing you match 2%, or hospital will match 3% of your voluntary contribution providing you match 3%.  Hospital will match 4% of your voluntary contribution, providing you match 4% or more.  Eligible after 1,000 hours and one year of service (hospital).  Enrollment in either July or January.

Income Protection

Income Continuation Plan (S&A): Accident and sickness benefits, hospitalization and maternity disabilities for up to 13 weeks paid at 70% of regular gross pay with a maximum of $700 a week.  Eligible first of the month following 90 days. 

Worker's Compensation: Provides medical care and percentage of income for on the job injuries.

Unemployment Compensation: Weekly payments up to certain maximums under the law if employed by the hospital and are then out of work through no fault of your own.  Immediately eligible.

Health Services 

Health Services: Periodic immunizations, hepatitis, flu vaccine, TB tests.  Immediately eligible.

Health Examination: Pre-employment physical, including lab work and TB test, ERGOS (depending on position).  Immediately eligible.

Employee Assistance Program (EAP): EAP offers free counseling on personal issues.  Immediately eligible.

Paid Time Off 

Vacations: 1-4 years = 10 days (full-time person); 5-9 years = 15 days (full-time person); 10 or more years = 20 days (full time person) per 2080 hours, after 1 year of service.

Holidays: Six paid holidays per year, after three months of service.

Paid Personal Days: Eligible after one year at service date, four days after one year (full-time).  Five days after four years (full-time).

Financial Services 

Payroll Deductions: Eligible for membership.  Specified savings institutions, Credit Union.  Immediately eligible.

Social Security: Financial assistance at retirement or disability.  Immediately eligible.

Leave of Absence 

Jury Duty: Regular pay minus jury duty pay.  Immediately eligible.

Military Leave: Granted a leave.  Immediately eligible.

Personal Leave of Absence: Time off for a period, generally not to exceed three months.  Immediately eligible.

Family & Medical Leave: Necessary time off to care for a family member, self, or a new child.  Eligible after 12 months and at least 1,000 hours of work.

Bereavement Leave: Regular pay for up to three days when there is a death in the family.

Education 

Educational Reimbursement Program: Educational reimbursement funds based on the categories of distribution.  Eligible upon hire; first work experience following completion of schooling. 

Boot Strap Program: Financial support for those accepted into a formal health careers program.  Need one year of service (2,080 hours of paid time).

Other Benefits 

Gift Shop: 10% discount on all items purchased in the hospital gift shop.  Provided by Partners of MMH&C.  Immediately eligible.

Service Awards:  Gift of recognition upon completion of 5, 10, 15, 20, 25, etc. years of service.

Rest Break: Two, 15 minute breaks per eight hour shift.  Immediately eligible.

Massage: Discount rate for massage through MMH&C Rehab. Department.

Yoga Class: Half-price for MMH&C employees.

Work Out Plus: Discount rate for MMH&C employees at fitness center.

Notary Public: Notary service available at MMH&C.
